What to Bring to Your First Appointment?
- New patient forms
- Photo ID and Insurance card
- Form of payment (cash, credit or debit)
- List of medications and doses
- List of supplements and doses—may bring in bottles or a picture of front and back of bottles
- List of Immunizations
- Name, address, phone of Primary Care Physician and any Specialists used
- Any testing you may have, especially most recent bloodwork, functional medicine testing, EKG, colonoscopy report, etc.

After Hours Care
If you are having an emergency, call 911 immediately. If you have a non-urgent matter that can wait until the next business day, a message can be left on the answering machine and our office staff will return your call the next business day. For all other matters, please visit an urgent care center near you.
Late Cancellation / No Show Policy
Effective August 1, 2018, a $50 “Late Cancellation / No Show” Fee will be charged to any patient that gives less than 72 hours notice for their previously scheduled appointment. This Self-Pay fee must be paid before another appointment can be scheduled. Although we do understand that emergencies happen from time to time, the office needs as much notice as possible to avoid any schedule vacancies.
Late Policy
In an effort to see patients on time and out of courtesy for fellow patients, you may be asked to reschedule your appointment if you are more than 15 minutes late for your appointment.
Blood Draw Policy
Monarch Integrative Medicine draws blood for the functional medicine testing we offer. The processing fee is $25.
Form and Letter Policy
There is a $25-75 charge for the completion of a form or letter requested. The price varies based on the complexity of the case and the amount of time to complete the form or letter.
Medication Refill Policy
Please notify your pharmacy when you need a medication refilled. Kindly allow 3 business days for your medication to be processed and refilled. Dr Kuhns generally prescribes enough medication to last until your next appointment. When you are on the last refill of your medicine make an appointment to follow up in the office and call the pharmacy for more medication.
If your dose has changed from the way it was prescribed on your bottle (either a different number of tablets or a different strength) please make the pharmacy aware of the change. They will then contact us with the medicine you are requesting and the correct dosage.
If you run out of medication on a weekend, the pharmacy may be able to give you a few pills to last until our office opens on Monday depending upon the medication. Otherwise you can be seen by a physician at an urgent care for a prescription refill.
Supplement Policy
As a convenience to patients, Monarch Integrative Medicine carries discounted supplements in the office and online. The supplements have been vetted to help you in selecting your products.
Unfortunately once supplements have been purchased in the office, they cannot be returned.
